As per Health Canada regulations, every time we receive a new Medical Document from your doctor we require a new Registration Application from you. Since the maximum length for a valid Medical Document is one year all clients will have to renew their prescription at some point should they wish to remain with Aurora. There are two simple ways to send us your new Registration Application:
1. You can find the form here. Under ‘Registration’, click ‘Print’ and the form will be automatically downloaded to your computer. Once it has been filled you can send it back to us via fax, email, or mail.

2. If you registered with an email on file, we will send you an online renewal form once we process your newest Medical Document. This message will contain a link and instructions that will allow you to complete an electronic application. (Please be advised that this is not the same as registering online.)
If you have an assigned caregiver, live in a non-private residence, or have opted for practitioner delivery, you cannot use the online renewal form and must submit the application using the first method.
After completing the above steps it is a good idea to contact Aurora either by phone or email to confirm your renewal – we want to make sure you have access to your medicine as quickly as possible! Once we have received your new Medical Document and Registration Application, our agents will promptly review your file. This process can take up to 5 business days.
You will receive an email as soon as you’re able to resume ordering medical cannabis from Aurora, or should any issues arise with your forms.
